What is the next step?

A. Remove suture.
B. Remove central line.
C. Prescribe broad spectrum antibiotics.
D. Prescribe steroids.
E. Reassurance

The correct answer is a. Remove suture.  There is a cellulitis around one of the sutures, which will be relieved with removal of the suture. Because the cellulitis does not effect the central line exit site, it is not necessary to consider removing the central line at this time. Broad spectrum antibiotics are not necessary, because this is a local irritation. Steroids are not appropriate to treat this issue. This should be addressed, because the irritation can become infected.
